How to Fill It

The IP Infringement Report Form is the official way to notify Elevate Golf when intellectual property may have been used without permission. Completing it clearly and accurately helps the claim to be reviewed quickly and fairly.

The form asks for contact details and the role of the person submitting the report, for example creator, rights holder, or authorised representative.

It then asks for a description of the original work, such as a video, image, logo, or written material, together with the basis for ownership.

The location of the suspected infringement on Elevate Golf should be provided, for example a profile, a Tip, or an uploaded file, with links where possible.

Proof of ownership should be included, such as links to the original, registration certificates, dated source files, or other clear evidence.

Finally, a declaration confirming the accuracy of the information and the authority to act is required.
